Aural Rehabilitation Referrals

Western’s Clinics accept referrals from doctors and other clinical experts. For some services, clients may refer themselves. 

Referrals may be faxed to 360-650-4334. Please let your referring provider know that since we do not bill insurance companies for services, the referral does not need to be approved by your insurance company first.  

Referrals are required for:

• Speech-Language Pathology services for both adults and children 
• Newborn Hearing Evaluations 
• Pediatric Hearing Evaluations 
• Balance/Vestibular Assessments and Testing 
• Auditory Processing Disorder Screening and Assessment 

Clients may self-refer for the following services:

• Gender-Affirming Voice Therapy 
• Adult Hearing Evaluations 
• Adult Hearing Aid Consultations 
• Cochlear Implant Candidacy Testing 
• Cochlear Implant Maintenance and Programming
